Student selection, curriculum, training environment, and special inducements should be considered in the attempt to increase the availability of primary care physician services to rural populations. Some controversial areas such as rural training sites, merging of primary care specialties, and compromise of the continuity experiences in residency are also addressed. Some basic philosophic differences in academic centers must be recognized.
